Professional Case Management

As members of the homeless community frequent the First Stop Day Center, they become aware of opportunities to engage with our Case Management team. Our full-time Case Managers work individually with clients on a detailed intake to identify and address obstacles to independence, and the reasons behind their homelessness. Next, Case Managers construct an Individual Service Plan to guide the client to essential services in our community, address outstanding legal issues, develop important life skills, obtain a secure income (via employment or participation in assistance programs), and ultimately obtain independent housing.

Beginning with helping clients obtain valid identification, our Case Managers take a step-by-step approach to tackle challenges, coordinating with partner agencies as needed to provide multidisciplinary support. The ultimate objective is to guide clients towards achieving independent, sustainable living.
